Health coverage in Indonesia

Healthcare has long been a challenge for Indonesia, and the country’s healthcare system is ranked 92nd place in the world by the WHO. Over the past few years, the government has taken significant steps to improve coverage and the quality of care. Nevertheless, most expats in Indonesia opt for private care, and some countries’ foreign ministries even recommend that their citizens leave the country in the event of a significant health issue.

How does the healthcare system work in Indonesia?

Under the Jaminan Kesehatan Nasional (National Health Insurance Programme) scheme, the Indonesian government plans to make basic medical care in public facilities free for all citizens. The goal of this programme, launched in 2014, was for 100% of the country to have coverage by 2019. However, progress has been slow. Not all hospitals have opted to join the JKN programme, meaning its effectiveness is still limited.

Expats working in Indonesia for six months or longer are required to sign up for the JKN programme. Registration is completed through the employer, after which employees and their dependants receive public health insurance cards.

Since private healthcare facilities typically offer better services and may have English-speaking staff, many foreigners opt for private health insurance, regardless of whether or not they are covered by the public system.

How to choose health insurance in Indonesia: local or international?

Due to the limitations of the Indonesian healthcare system, it is important for expatriates to choose a health insurance package that not only allows them to access the private healthcare system but also to be repatriated to neighbouring countries or to their home country in case of a medical emergency or need for specialised care. This is a guarantee that local private health insurance does not offer, and thus requires the purchase of international health insurance.

In addition, the use of medical telemedicine services is an effective solution for primary care needs, allowing you to speak to a qualified healthcare professional in the language of your choice anytime, anywhere.

Choosing hospitals in Indonesia?

The Indonesian healthcare system is made up of hospitals, clinics, and Puskesmas (state-run community health services), but these are often overburdened and understaffed. On the other hand, private hospitals in Indonesia offer a significantly higher quality of care than the public system.

Most of the best hospitals in Indonesia are located in Jakarta, the capital. However, in case of serious health needs or very specific treatment, evacuation to a better equipped neighbouring country is often recommended, for instance to Singapore.
